Description: Thamesview Family Health Team provides comprehensive, patient-centred primary health care including chronic disease management and health promotion and prevention, to a rostered population of patients in the Chatham-Kent region.

Interdisciplinary Team: Dietician; NP’s; Occupational Therapist; Pharmacist; RN’s; Respiratory Therapist; Social Workers

Site Locations: 2 locations in Chatham; 1 location in Wheatley
FINANCE
Compensation Type: Blended Capitation (FHO)
Est. Annual Salary Range: Varies – it depends on patients rostered and you should be able to get exact numbers from your college.
Overhead Type / Amount: Fixed Overhead- Call to inquire ($8,000-$10,000 and is subject to change)
Admin Staff: TFHT will support physicians in finding an admin staff (unless POD they join has one). Admin staff are trained and monitored by TFHT Admin
Other compensation/incentives/perks: CK Task Force 1 time incentive $25,000
PATIENT INFORMATION
Patient: Take over an existing roster; build a roster; absorb unattached patients
Est. Patient Roster Size: 700 – 2000
Pt Demographics: Participation in urgent care clinics is mandatory as per the Ministry of Health
Est. Weekly Pt Hours: flexible- 35 hours per week

Onboarding: 1-4 days orientation to EMR and team as well as on-going support
